Clinical Observation of Pneumonia Caused by Klebsiella Pneumoniae in Adults


Abstract
A clinical observation was made on 70 cases of Klebsiella Pneumonia, who were admitted to Chungnam National Univ. Hospital, Dae Jeon and National Medical Center,
Seoul, from January, 1971 to December, 1976.
1. In age and sex distribution, high incidence was noted in the group of fifth decade and the male to female ratio was 3. 4 : 1.
2. Fever and chill were the most common symptoms in this study and productive coughing, dyspnea, chest pain, epigastric pain and hemoptysis followed in order of frequency.
3. The predisposing factors and/or associated disease were found in 82.9% of patients and disturbance of consciousness was the most common predisposing factor and pulm¡þonary tuberculosis was the most common associated disease.
4. The most fre4uently involved area of the lung on the chest X-ray was the right upper lobe (37. 1%).
5. Leukocytosis (above 10, 000/cu.mm) was found in 62.8% and leukopenia (below 5,000/cu.mm) in 8. 6% of the patients.
The marked elevation of ESR(above 50mm/ hr.) was noted in 46 cases(65. 7¡Æ%) and
normal range of ESR(below 10mm/hr.) in 4 cases(5.7%).
6. The highest monthly incidence was noted in April(20.00%).
7. Growth of organisms other than Klebsiella in sputum culture was noted in 24
cases, among which E. Coli was the commonest one, occupying 33.3% of 24 cases.
8. Klebsiella was most sensitive to kanamyin among eight antibiotics.
9. Frequency of complications was 48.6% and pleural effusion was the commonest one.
10. Eight patients died of the disease and the over all mortality rate was 11.4%.
